Important: If you intend to use your insurance (Aetna, MHN), please contact your insurance carrier before the first session to ensure that your provider’s services are covered by your plan. At the current time, CAPP is contracted only with Aetna Behavioral Health and Managed Health Network (MHN). Please contact your doctor about provider-specific forms.

Please note that uninsured/self-pay patients have a right to a good faith estimate of costs for scheduled sessions. Please contact your doctor if you have any questions about current rates or would like to obtain a good faith estimate of costs. Some of the cost information may be in the office policy document (updated 1-1-22)
